Position Overview:

We are seeking a dedicated and compassionate Physical Therapist Assistants (PTA’s) and Occupational Therapist Assistants (COTA’s) to join our home care team. The ideal candidate will work under the supervision of our licensed Physical Therapists to provide individualized therapeutic services to patients in their homes. This role requires an individual who can drive to patient’s homes and foster meaningful relationships with the patients, ensuring they receive the highest standard of care in a comfortable, familiar environment.

Key Responsibilities:

• Patient Care: Assist in executing individualized treatment plans designed by the Physical Therapist to help patients regain mobility, strength, and independence.
• Documentation: Maintain accurate patient records, noting progress, changes, and any potential concerns.
• Communication: Liaise regularly with the supervising Physical Therapist about patient care, providing updates and receiving feedback.
• Home Safety: Evaluate patients’ homes for potential hazards and educate patients and families on home safety and any necessary modifications.
• Patient Education: Instruct patients and their families on exercises, mobility aids, and equipment, ensuring they’re used safely and effectively.
• Equipment Maintenance: Clean, maintain, and ensure proper storage of therapeutic equipment and tools after home visits.
